1. Intake Form (To collect all the important information about you according to Modern aspect and Ayurvedic aspect)

2. Prakriti Form (To find your Ayurvedic Body type)

3. Vikriti Form (To find the current disturb state of your body and energies in your body)

You will undoubtedly find these forms to be quite in-depth; this is because Ayurveda is holistic and a complete understanding of you, your health history, lifestyle patterns, and health concerns is an important part of the process.

At any stage you can save and exit the forms.

Try to fill the forms as much as you can, otherwise whatever section is left we will either ring you or you make an appointment at no extra cost for one hour ( time after one hours will be charged as $75/hr)  and discuss your unique treatment plan.
